New data shows significant reduction in hospitalizations

Nearly 18 months into our partnership with Wellpoint (formerly known as UniCare) — and more than halfway through the enrollment period of a clinical trial study that seeks to prove its benefits — new data suggest Ibis Health is making significant headway in keeping program members out of the hospital and healthier at home.

The analysis of acute care utilization data shows that after joining the program, Ibis Health members experienced 10 percent fewer emergency room visits, a 30 percent fewer hospital admissions, 50 percent fewer days spent in hospitals and 55 percent fewer repeat admissions after they were discharged. Evidence also suggests that our members’ chronic conditions progressed more slowly than those who chose not to participate in the program.

Ibis Health launches the Ibis Passport!

Take your health information on the go with the Ibis Passport. Now you can share your health data with your doctors or your family members whenever you need to - so they can help keep you healthy!

The Passport is the size of a credit card and it allows members to access their self-reported vital health data on the go, creating an easy way for them to share it with medical providers or family members. Now doctors can quickly assess their patient's status and progress, making visits more efficient for our members and their providers.

UniCare partners with Senscio Systems for GIC members

UniCare Offers Innovative Program to Reduce Hospital Admissions and Improve Health Outcomes

UniCare is now offering an innovative, at-home solution to manage chronic conditions as part of its continuing efforts to improve the health of its members by simplifying the healthcare experience. Through its partnership with Senscio Systems, UniCare retiree members will have access to a new virtual chronic care management program called Ibis Health that supports each individual’s whole health through daily connected care, round-the-clock monitoring, and one-to-one monthly support.
